Conduct intake assessments and develop individualized service plans addressing housing, income, medical, mental health, and social support needs Provide referrals and linkages to supportive resources across housing, health, and social services Complete program-specific assessments including the VI-SPDAT Coordinate care with medical providers, social workers, and housing partners Maintain accurate, HIPAA-compliant documentation and case files Conduct 90-day progress reviews with participants
Employment authorization to work in the U.S. Valid California driver's license, reliable vehicle, and proof of insurance CPR/First Aid certification (or willingness to obtain upon hire) Must pass tuberculosis test, background screening, and drug test
2+ years of experience in healthcare, case management, homeless services, or vulnerable population support

Position Overview
Join our Orange County Care Coordination Team (Health - OC) as a CalAIM Community Supports Case Manager. You will focus on member housing navigation and retention, collaborating with a multidisciplinary team to support medical and housing stability for CalAIM participants.
