2+ years of experience in healthcare, case management, homeless services, or vulnerable population support

Position Overview
PATH seeks a compassionate Case Manager to join our Santa Clara County team at the Bristol Hotel Emergency Shelter. You will help clients break the cycle of homelessness using a "whatever it takes" approach, supporting individuals through street outreach, interim housing, and into permanent housing.
