Employment authorization to work in the U.S. Valid California driver's license, reliable vehicle, and proof of insurance CPR/First Aid certification (or willingness to obtain upon hire) Must pass tuberculosis test, background screening, and drug test
2+ years of experience in healthcare, case management, homeless services, or vulnerable population support

Position Overview
Join PATH's Care Coordination Services team in Orange County. As a Case Manager, you will assist clients in breaking the cycle of homelessness by utilizing a "whatever it takes" approach, supporting people across all housing phases from street outreach through permanent housing placement.
