Position Overview
Join PATH's Health - PSS Team in Los Angeles as a CalAIM Enhanced Care Management Case Manager. You will focus on member medical care coordination, assist in navigation of the healthcare system, and ensure medical and housing stability for high-acuity CalAIM participants.
