Minimum 2 years' experience working with homeless populations, preferably those with mental illness or substance use disorders Case management or mental health services background Superior writing and communication skills Proficiency with HMIS, MS Word, and Excel HIPAA compliance knowledge Valid driver's license and reliable transportation Background check and fingerprinting required

About the Role
Intensive Case Management Services (ICMS) Case Manager working with formerly homeless individuals who have chronic medical conditions and co-occurring disorders. The role focuses on mitigating housing loss risk by enhancing clients' ability to problem-solve and coordinate services using motivational interviewing, harm reduction, and trauma-informed approaches.
Salary: $23.69 – $27.87/hour | Full-time (40 hrs/week)
