About the Role
Intensive Case Management Services (ICMS) Case Manager working with formerly homeless individuals who have chronic medical conditions and co-occurring disorders. Focuses on housing retention through strength-based, trauma-informed case management using motivational interviewing and harm reduction approaches.
Salary: $23.69 – $27.87/hour | Full-time (40 hrs/week)
