About the Role
Intensive Case Management Services (ICMS) Case Manager working with formerly homeless individuals — including transitional youth — who have chronic medical conditions and co-occurring disorders. Uses harm reduction, critical time intervention, and stages-of-change methodologies to help clients maintain housing and manage complex needs.
Salary: $23.69 – $27.87/hour | Full-time (40 hrs/week)
